1. Date of execution of the contract

2. Parties to the contract: the creditor (da'in) and the debtor (madin)
- Full name of the debtor (madin)
- debtor's phone
- Full name of the lender (da'in, the creditor)
- lender's phone
3. Subject matter of the contract and amount of the debt
- amount in figures
- amount in words
- Currency
4. Term for repayment of the debt (ajal)
- Repayment term: day, month, year
5. Securing the debt by pledge (rahn): possession (qabd) and liability (daman)
- Pledge (rahn): description, identification, valuation
- Has the pledge been delivered into possession — qabd (yes/no), and date
- Who bears the maintenance costs and the risk of loss (daman)
6. Attestation of the debt (ishhad)
- Witness No. 1 (full name + phone)
- Witness No. 2 (full name + phone)
- Witness No. 3 (full name + phone)
Recording the debt in writing and having it witnessed: by ijma' (scholarly consensus) the loan is valid even without them (Ibn Hazm, «Maratib al-Ijma'»), yet verse 2:282 expressly commands that debts be put in writing, and part of the salaf held neglect of this to be sinful.
