What the form contains

- Consent of the bride: how it was given and who witnessed it
- Ijab and qabul (offer and acceptance) pronounced in one sitting (yes/no), and by whom
4. Mahr and manner of payment
- Mahr (bridal gift): what exactly and how much
- To whom the mahr was handed over (if paid immediately)
- Due date of the mahr (if deferred)
5. Additional stipulations and obligations
- Additional stipulations and obligations
6. Witnesses
- Witness No. 1: full name
- Witness No. 2: full name
This document records the conclusion of the marriage contract between the parties. A marriage certificate is issued by an authorised person or body; this form replaces neither that certificate nor state registration.
The two witnesses attest to their presence at the contracting and at the wali taking the consent of the woman under his guardianship.
Once the mahr has been handed over, it is advisable to record this on the same document with the date and the signatures.
